We Welcome a Family-Centered Model of Care

 

Each family or individual undergoes an initial assessment to determine the most appropriate services. Our highly qualified and extensively trained caregivers are matched to clients and regularly supervised. Daily activity and care giving notes are provided for the families to keep informed of their loved one’s mental and physical condition.

The sky is the limit.  We pride ourselves on creating unique service plans that meet the needs of our family members.

 

Match Caregiver to Family

Using our detailed assessment and experience, we match the perfect caregiver to each family.  Recurring background checks, drug-screenings and motor-vehicle inquiries (when applicable) are required for all of our caregivers.

 

Supervise

We regularly visit our families' homes to observe and advise our caregivers.  Supervision assures that you don't have to worry that your loved one is receiving the highest level of care available.

Communicate with Family

You'll be in-the-know as we provide regular updates on your loved one's health, mood, activities and more.  If there is a notable change or emergency, you are contacted immediately.

Modify Service Plan

  We re-assess every client and adapt service plans to meet the changing needs.  We do this by careful and considerate collaboration with families and the broader care network.
